This is a boost mode (step up) LED driver for series strings of 1W LEDs, you can drive from 3 up to 15 leds in series, delivering 280-300mA
The input voltage range depends on the number of LEDS, for 10 - 15 leds, you can supply 12v to 30v DC, for 9 and fewer leds see the image which shows the voltage ranges.

Input and output are solder pads, solder wires to them. Inputs are marked "V+" and "V-". Outputs are marked LED+ and LED-, these are polarised of course.
The driver appears to be the XL6005E1 from XLSEMI.
